POLICY:
Our Heart Home Care provides all services and referrals relative to patient care and employment actions to any eligible and qualified individual regardless of age, race, sex, national origin, religious creed, handicap or sexual preference. The nondiscriminatory policies of the agency apply to all patients/families served, employees and contractors.
PROCEDURE:
1. As a recipient of Federal financial assistance, Our Heart Home Care does not exclude, deny benefits to, or otherwise discriminate against any person on the basis of age, race, sex, national origin, religious creed, handicap or sexual preference in admission to, participation in, or receipt of the services and benefits under any of its programs and activities, whether carried out by Our Heart Home Care directly or through a contractor or any other entity with which the agency arranges to carry out its programs and activities. This statement is in accordance with the provisions of Title VI of the Civil Rights Act of 1964, Section 504 of the Rehabilitation Act of 1973, the Age Discrimination Act of 1975, and Regulations of the U.S. Department of Health and Human Services issued pursuant to these statutes at Title 45 Code of Federal Regulations Parts 80, 84, and 91.

2. Patient care will be provided in a manner that does not discriminate against persons regardless of age, race, sex, national origin, religious creed, handicap or sexual preference.
3. Staff, whether employees or contractors, will be hired regardless of age, race, sex, national origin, religious creed, handicap or sexual preference.
4. Reasonable accommodation will be provided for any handicapped employees.
5. Staff will be assigned to patient cases regardless of age, race, sex, national origin, religious creed, handicap or sexual preference.
6. All possible issues of discrimination should be reported immediately to the Administrator.
7. All patients and agency staff/contractors are made aware that complaints of potentially discriminatory behavior may be filed with the Office of Equal Opportunity, Pennsylvania
